How do I treat dandruff and seborrheic dermatitis of the scalp?

Unfortunately, there is no magic "cure" for these conditions. However, most symptoms can be controlled and treated with over-the-counter medicated shampoos. These shampoos use three different approaches for symptom control:

  • Slow down the process: Shampoos with coal tar help slow the proliferation (overproduction) of skin cells on the scalp, and reduce potential inflammation.

  • Clear scalp buildup: Shampoos with salicylic acid help break down scalp buildup and split flakes into smaller, less visible pieces that can be washed away easily. A shampoo with salicylic acid can be used as a primary shampoo or as a first step in a scalp treatment regimen, especially for those with significant flaking and buildup.

  • Remove the yeast: Shampoos containing pyrithione zinc, ketoconazole, or selenium sulfide remove yeast, a potential scalp irritant, from the scalp.

How do I treat dandruff and seborrheic dermatitis of the scalp?

If your current medicated shampoo is not controlling or improving your symptoms, try switching to one with a different active ingredient. Dermatologists may recommend “rotational therapy”—using specific products on alternating days to attain the best results when treating scalp conditions. For example, severe scalp symptoms may be treated by shampooing twice a week with a product containing salicylic acid to clear flake and sebum buildup, and shampooing on other days with a product containing coal tar to control flake formation. Always follow the instructions provided with medicated shampoos and conditioners. Once symptoms clear, weekly use will help prevent further flare-ups.

Occasionally, severe scalp conditions may require treatment with a prescription-strength shampoo, cream, or ointment. Your dermatologist can provide you with specific advice on treating serious scalp problems.
